Overview:
The My Diabetes Health Assessment is a tool to determine a diabetic person's risk of heart attack and stroke. The program is an online, interactive, personal risk assessment tool specifically for people with Type 2 Diabetes. After completing the My Diabetes Risk Assessment, users are encouraged to print the personalized report and bring it directly to their health care provider to develop a personal plan.
